After booking your appointment, you'll come here to complete your release form.
Release forms must be submitted prior to your appointment date.
Complete this release form if you have booked an appointment for a new piercing and you're at least 18 years old.

You'll only need to fill out this form once per appointment, even if you're receiving multiple piercings, but you do need to complete a new form for each separate visit.
Complete this release form if you've booked an appointment for a new piercing for a person under 18 years old.

You'll only need to fill out this form once per appointment, even if your minor child is receiving multiple piercings, but you do need to complete a new form for each separate visit.
Complete this form if you've booked an appointment for a service that IS NOT a new piercing. Use this form for jewelry changes/installs, downsizes, consultations, troubleshooting etc.

Complete this release form IN ADDITION to the "new piercing" release form if you have booked a piercing service with our apprentice. Only clients 18+ are eligible to be pierced by our apprentice piercers.
